FERPA Forms
The Family Educational Rights and Privacy Act (FERPA) protect the privacy of student education records. In compliance with FERPA regulations, only the student to whom the education records pertain to may authorize the release or withholding of such records. Accordingly, a FERPA Form must be completed, signed, and submitted by the student in order to authorize the release or withholding of their education record. A FERPA Form may not be completed or submitted by a parent, guardian, or any other individual on the student’s behalf.
Consent to Release Information
The FERPA Consent to Disclose Form authorizes the institution to release specified educational and/or financial information to individuals designated by the student.
- Educational information may include, but is not limited to: grades, academic standing, grade point average
(GPA), attendance records, and any other details contained within the student’s education
record.
- Financial information may include, but is not limited to: financial aid status, student account and billing
information, and any other data related to the student’s financial records.
This consent must be provided voluntarily by the student and is required before any such information may be disclosed to third parties, including parents, guardians, or other individuals.
Step 1: Log into your Navigate account
Step 2: Click on Resources - Places
Step 3: Select the Forms folder and click on “Complete FERPA form”
Step 4: Click on the link displayed to open a new window
Step 5: Log into YTC eForms using your Navigate User Id and Password
Step 6: Under the Document Library, click the “FERPA Consent Form” form and complete the fields contained within the form
Step 7: Once fields are completed, click send
In compliance with FERPA, York Technical College verifies directory information as requested by third parties. Directory information consists of the following information:
- Student Name
- Address
- E-mail Address
- Telephone Number
- Dates of Attendance
- Full-Time/Part-Time Status
- Program of Study
- Anticipated Graduation Date
- Awards
- Honors
- Degree, Diploma, or Certificate Conferred
- The student and any outside party who has the student’s written consent.
- School officials (as defined by York Technical College) who have a legitimate educational interest.
- Parents of a dependent student as defined by the Internal Revenue Code.
- A person in response to a lawfully issued subpoena or court order, as long as the college makes a reasonable attempt to notify the student first.
York Technical College adheres to the rules and regulations of the Family Educational Rights and Privacy Act (FERPA) to protect the privacy of student information; however, students also have a responsibility to protect their information. Students access college computing systems such as Desire2Learn and Self Service via login information (username and password). Students should NOT share their login information. Releasing college system login information to others may be considered a violation of the Student Code of Conduct. Students who wish to prevent the release of their directory information, must complete the FERPA Do Not Disclose Form through Navigate. Submitting this form will place a non-disclosure request on the student’s record, which will remain in effect indefinitely unless the student submits a written request to the Registrar’s Office to have it removed. By completing the FERPA Do Not Disclose Form, York Technical College will be prohibited from releasing the following directory information without the student’s prior written consent:
- Student Name
- Address
- E-mail Address
- Telephone Number
- Dates of Attendance
- Full-Time/Part-Time Status
- Program of Study
- Anticipated Graduation Date
- Awards
- Honors
- Degree, Diploma, or Certificate Conferred
Important: Students are strongly encouraged to consult with the Registrar’s Office before submitting the FERPA Do Not Disclose form to fully understand the potential consequences of restricting access to their directory information.
Request to Withhold Information
Under the Family Educational Rights and Privacy Act (FERPA), students also have the right to withhold the disclosure of their education records, including directory information. By submitting a FERPA Do Not Disclose Form, the student instructs the institution not to release any information from their education records, including directory information, without the student's prior written consent.
Students wishing to withhold their information must complete, sign, and submit the appropriate request form. This request remains in effect until the student provides written notice to revoke it.

Complete the FERPA Do Not Disclose Form
Completion of the FERPA Do Not Disclose Form prevents York Technical College from releasing directory information such as:
- Student Name
- Address
- E-mail Address
- Telephone Number
- Dates of Attendance
- Full-Time/Part-Time Status
- Program of Study
- Anticipated Graduation Date
- Awards
- Honors
- Degree, Diploma, or Certificate Conferred
A FERPA Do Not Disclose request remains on a student’s record indefinitely, including after graduation or withdrawal from the institution, unless the student submits a signed, written request to the Registrar’s Office to have it removed.
To remove a previously submitted FERPA Do Not Disclose request, the student must submit a signed, written request to the Registrar’s Office indicating their desire to remove the non-disclosure status. This request must include the student’s full name and identifying information, such as student ID number, for processing. Once processed, the student’s directory information may again be released in accordance with FERPA regulations.
