FERPA Forms

The Family Educational Rights and Privacy Act (FERPA) protect the privacy of student education records. In compliance with FERPA regulations, only the student to whom the education records pertain to may authorize the release or withholding of such records. Accordingly, a FERPA Form must be completed, signed, and submitted by the student in order to authorize the release or withholding of their education record. A FERPA Form may not be completed or submitted by a parent, guardian, or any other individual on the student’s behalf.

Consent to Release Information

The FERPA Consent to Disclose Form authorizes the institution to release specified educational and/or financial information to individuals designated by the student.

  • Educational information may include, but is not limited to: grades, academic standing, grade point average (GPA), attendance records, and any other details contained within the student’s education record.

  • Financial information may include, but is not limited to: financial aid status, student account and billing information, and any other data related to the student’s financial records.

This consent must be provided voluntarily by the student and is required before any such information may be disclosed to third parties, including parents, guardians, or other individuals.

Request to Withhold Information

Under the Family Educational Rights and Privacy Act (FERPA), students also have the right to withhold the disclosure of their education records, including directory information. By submitting a FERPA Do Not Disclose Form, the student instructs the institution not to release any information from their education records, including directory information, without the student's prior written consent.

Students wishing to withhold their information must complete, sign, and submit the appropriate request form. This request remains in effect until the student provides written notice to revoke it.
